Support

Our resources page contains some information about how to make a Change of Conditions application yourself.

We will be closed to new referrals until at least the end of April, while we work through our waiting list.

If you would like us to email you when we reopen for new referrals, please complete the short form below. This is not a referral form for support from The Unity Project. When we reopen, we will email you to invite you to complete a referral form.
